SWEEPSTAKES SUMMARY: This national sweepstakes promotion (“Promotion” or “Sweepstakes”) is sponsored by the Health-Mor division of HMI Industries Inc. The Promotion is governed by these Official Rules. By completing and signing an entry form, you agree that the Sponsor and co-sponsors of this Promotion may telephone you, even if your number is found on a statewide or national do not call list or registry. You may be required to confirm information you have provided on the entry form. Completing an entry form may result in a sales solicitation for FilterQueen home care/indoor air quality products made by the Sponsor. You are not required to view any sales demonstration or to purchase anything. The purchase of merchandise will not improve the chance of winning. 2. HOW TO ENTER: Hand print your full name, address, phone number and Email address (if applicable) on the official entry form (no mechanical or photocopy reproductions permitted). Otherwise, fully complete and sign that form; and then deposit the signed form in the entry box, or give the signed form to your authorized FilterQueen distributor, or mail it to the Sponsor at the address above. In addition, you will automatically be entered in the Sweepstakes if, in accordance with the FilterQueen “Instant Neighborhood” promotion, you supply a lead to a prospective customer to whom a live commercial of FilterQueen home care products is performed within 72 hours. Every such completed lead results in an additional entry into the Sweepstakes. You may also enter one time online through www.FilterQueen.com/sweepstakes. Telephone entries are not permitted. 3. ENTRY LIMITATIONS: Entry forms and online entries must be received by 5:00 p.m. EST on January 29, 2010. Entries become the Sponsor’s property. This Promotion is open only to legal U.S. residents 18 years of age and older, who are not employees, representatives, agents or suppliers of the Sponsor or co-sponsors, their subsidiary or affiliate companies, nor members of their immediate families. Sponsor is not responsible for printing errors or inaccurate, incomplete, stolen, lost, illegible, mutilated, postage due, misdirected, delayed or late entries or mail, or equipment or telephone malfunction. By signing an entry form, making an online entry, or participating in the Instant Neighborhood promotion, you are formally representing that you are eligible to enter this Promotion and are bound by these Official Rules. 4. PRIZE: This Promotion has one prize, $20,000 in cash in one lump sum payment (”Prize”). Estimated retail value: $20,000. Winner is responsible for all federal, state, and other taxes and charges. The Prize will be awarded. Odds of winning depend on the number of eligible entry forms received, added to the number of eligible online entries, added to the number of eligible entries credited through the Instant Neighborhood promotion. 5. TIME PERIOD AND SELECTION OF WINNER: Sweepstakes begins on February 1, 2009 and ends at 5:00 p.m. EST on January 29, 2010. Sponsor will thereafter randomly select one winner from eligible entries that were received in time. The winner will be selected in a random drawing held by February 15, 2010 and will be notified by February 26, 2010. 6. REQUIREMENTS OF WINNER: The winner must prove eligibility and will be required to sign an affidavit of eligibility and release. To receive the Prize, the winner must grant written permission to have his or her name used for Sponsor’s advertising and promotional purposes without additional compensation. The failure of a potential winner to prove eligibility or sign any required affidavit or grant of permission, or inability of the Sponsor to contact a potential winner, will result in disqualification and selection of an alternative winner. 7. RESERVATIONS: Void where prohibited or restricted by law and subject to all laws and regulations. By entering this Promotion, each entrant agrees to be bound by these rules, and the decisions of the Sponsor which are final and not subject to challenge. Sponsors and co-sponsors are not responsible for claims, injuries, losses or damages of any kind resulting from the acceptance, use, misuse, malfunctioning, possession, or loss of the Prize. 8. POSSIBLE DAILY GIFT OFFERS: You may be selected by a co-sponsor and may be eligible to receive a daily thank-you gift subject to viewing a 60+ minute product display of FilterQueen home care/indoor air quality products. You will be under no obligation to do so, and agreeing to do so will not improve your chance of winning the Sweepstakes. The estimated retail value of any such gift will be less than $50.
